These are the steps you need to take to be eligible for the Minnesota Medical Marijuana Program.

You may be eligible if you have an Online Medical Marijuana Card from https://www.420id.com/ and you have been diagnosed by a registered medical practitioner with the program with at minimum one condition.

  • Intractable discomfort

  • Post-Traumatic Stress Disorder (PTSD) is a type of post-traumatic stress disorder.

  • Autism

  • Obstructive sleeping apnea is a type of obstructive sleep disorder.

  • People are affected by Alzheimer's disease.

  • Long-lasting pain

  • Sickle Cell Disease can be described as a type of sickle cell disease.

  • If you experience severe or persistent pain, nausea, or severe vomiting due to your condition, treatment, or other factors,

Certify

A doctor, physician assistant, or advanced practice registered nursing must verify that you are healthy before you can apply to the program.

  • Call your doctor, physician assistant, or advanced practice registered nurses to schedule an appointment.

  • The OMC does not keep track of participants in the program. You will be responsible for booking an appointment with licensed healthcare providers.

  • Download and print the Patient email and acknowledgment form (PDF).

  • Please complete the following fields to access the Acknowledgement or Patient Email Forms

  • Please enter the email address you wish to receive confidential information about your health.

  • Bring this form to your appointment. Hand it to your doctor.

  • Ask for a summary of your experience and a list of all of the medications you have taken during that visit.

You will receive the following information from OMC after your appointment with your doctor:

  • Confirmation by your doctor.

  • Unique registration link to the Medical Cannabis Patient Registry

Register

After you receive your certificate email, you can register. Gather the following information before you click on the Register button:

  • Some examples of government-issued ID are state ID, driver’s license, and passport.

  • To be eligible for the discount fee, proof must be provided that you are receiving Supplemental Security Income, Social Security Disability (including those who have been transitioned into retirement), medical aid, MinnesotaCare, IHS, Railroad Disability or VA dependency, indemnity compensation, or Veteran’s Disability benefits.

  • Take photos with your IDs using a smartphone equipped with a camera.

  • The annual registration fee can be paid by check, credit/debit card, or cash. All payment methods are accepted by U.S. Bank

  • Once your account has been approved, you will be sent an email. Next, you can proceed to Step 3: Receive.

Redeem

Before you can purchase medical cannabis, please complete a Patient Evaluation.

Go to:

  • Login to your Medical Cannabis Patient Registry account.

  • Every time you want medical cannabis, you must submit a patient self-evaluation.

  • Bring your appointment summary and a list of medications you have been prescribed by your doctor to a Cannabis Patient Center.

You will be able to meet with a qualified pharmacist at the Cannabis Patient Center.

  • Take a look at your self-evaluation and practitioner appointment summary. Also, take a look at the medications you are taking.

  • Create a unique recommendation for medical cannabis dosage.
